CBD and Prader Willi Syndrome

Prader Willi Syndrome (PWS) is a genetic disorder that occurs in approximately one out of every 15,000 births.

PWS affects males and females with equal frequency and affects all races and ethnicities. PWS is recognized as the most common genetic cause of life-threatening childhood obesity.

PWS was first described by Swiss doctors Andrea Prader, Alexis Labhart, and Heinrich Willi in 1956 based on the clinical characteristics of nine children they examined. The common characteristics defined in the initial report included small hands and feet, abnormal growth and body composition (small stature, very low lean body mass, and early-onset childhood obesity), hypotonia (weak muscles) at birth, insatiable hunger, extreme obesity, and intellectual disability

How CBD May Help Prader Willi

The human ECS plays a regulatory role in endocrine and metabolic functions, along with the central nervous system, for appetite control, food intake, and energy balance. A 2011 study in the journal of Pharmacology and Biochemical Behavior found that CBD can act as an agonist for CB1R by blocking the uptake of the human endocannabinoid molecule anandamide. There are multiple systems collaborating for food intake control including both the cannabinoid and serotinergic pathways. Williams and Kirkham (2002) published research focused on the role of SR141716A (rimonabant), a CB1R antagonist, which was shown to reverse the hyperphagic effect caused by the phytocannabinoid delta-9 tetrahydrocannabinol (THC). Rimonabant suppresses food intake signaling; CBD is an alternative cannabinoid that has similar antagonistic effects as rimonabant though may target different receptors than CB1R. CBD on its own may not change food intake but may prevent or disrupt the physiological process leading to hyperphagia.

PWS is an orphan disease with major endocrine and behavioral manifestations and no FDA-approved therapies for the treatment of hyperphagia. Caregivers of these patients often prioritize hyperphagia, the most common genetic cause of life-threatening childhood obesity, and anxiety as the most important symptoms for treatment of the disease. From existing literature and previous clinical trial data generated, cannabidiol has a multifactorial mechanism of action showing clear biological activity against both of these symptoms.

Dosing Thoughts

Research studies have used doses from 15mg to as high as 600mg. Typical CBD supplements provide 20mg to 25mg. When taking CBD for lupus pain, dosing can vary widely depending on the depth of pain you are experiencing. One goal to consider is to use CBD to replace pharmaceuticals you might be taking. CBD is an all-natural alternative that does not build up in your system and cannot be overdosed.

It is important to consult with your healthcare provider before starting any supplement program. Most prescribed medications cannot and should not be stopped all at once.
It has been suggested to start with a dose of 10mg to 15mg of CBD twice a day. After a few days, you may want to reduce prescribed meds incrementally while increasing your CBD dosage the following day. Steps of 10mg are suggested to help monitor changes. Monitor your symptoms; be aware of how you feel. Don’t hurry the process. Wait 2 to 3 days between changes. This will allow you to determine the dosing that your body requires to meet your goals, while documenting how your medications interact.
